I suppose what I'm asking is, how did you conclude the Netcraft
toolbar is the source of the leakage?

Because Netcraft says that it collects the URLs visited using a
browser with Toolbar installed. See <

And the intrAnet sites that I saw on the netcrafts  listing, are not
listed anywhere on the internet, and are not refering to any outside
websites.
