I am actually pursuing the ISSAP as well. I was able to find the
following FAQ on the www.isc2.org website, included below. To answer
your first question, per the FAQ, you should be able to take this
examination at any of the published testing events. I was able to find a
US price of $349. If this follows the CISSP exam, it will probably cost
you between $399 & $449. I haven't been able to find any recommended
reading yet, but I will forward it on when I find some.

Each exam lasts 3 hours and costs $349 USD. There are savings if you
register for two or more exams. Moreover, "the ISSMP and ISSAP
concentration examinations are comprised of 100 scored items plus 25
pretest items - for a total of 125 items. The ISSEP concentration
examination is comprised of 125 scored items plus 25 pretest items - for
a total of 150 items."
