That depends on the software and version you're using. If you're using OpenSSH, you need 4.0x or higher, and it's specified as an optional listen address before the listen port in forward specification, eg


ssh -L 192.168.1.1:1234:192.168.2.100:5678 someserver

will listen on 192.168.1.1 port 1234 and forward to 192.168.2.100 port 5678.

Other software may have similar options.
