If I scan by ip address range, how do I get the ip address and the
computer name to appear on the html report?  I'm running from the
command line and creating an nbe file and then converting to html.

Under the general category, there's a "host FQDN" (fully qualified domain
name) plugin.  Does including that in your scan help?
