Do subsequent Nessus scans repeatably miss the XEXCH50 flaw too?
One other thing I forgot to mention, you might want to take a look in the kbs to verify that the 'SMTP/'+port+'/broken' (where port is the SMTP port) item does not exist and verify that the port was recognized and accessible during the scan.
