1- I have read from the website that it is a six hour
test with 250 questions. The test is paper based.
Is there any break within this six hour period?

Nope.  Security geeks are tough!

(There is no official break.  According to the exam rules, one person at a time 
may be allowed to take a break at the back of the room.)

Is
there no computer based test for this?

No computer-based exam system has been found with the necessary level of 
security.

2- Is there any laboratory examination or just a 
paper test only?

Paper only, 250 multiple choice questions.

3- For how long is this certification valid? Do I need
to keep taking the test every couple of years to
keep the certification?

Once you pass the exam you do not need to retake as long as you keep your 
credentials up to date.  There is a requirement for continuing professional 
education.
