[CISSP-D] Best Practice procedures for incident response and patch manag

Subject: [CISSP-D] Best Practice procedures for incident response and patch management

Are there any good sites that have suggested procedures, including 
example documents and such, regarding incident response and patch 
management for a small to medium organization??  I have looked at the 
SANS reading room, ISF and many others and cannot find examples of a 
step-by-step approach with example documents.  I would prefer to 
start with something that is workable rather than re-inventing the 
wheel.  Any suggestions?
